The purpose of this survey is to determine how we can work together to develop happy and healthy students and families. It will be sent out again at the beginning of June so that results can be compared and to determine if any progress has been made.

The survey has four components with specific questions and are defined as follows:

1. Physical Health component consists of 4 questions

2. Nutrition component consists of 4 questions

3. Mental Health component consists of 3 questions

4. Comments and/or suggestions

For those questions with rating from 1 to 4, please consider 1 = Excellent, 2 = Good, 3 = Average, and 4 = Below Average
